PATIENTS’ BILL OF RIGHTS*
As a patient at Chicago-Read Mental Health Center you have certain rights, which include:
1. The Right to treatment without being discriminated against because of race, color, religion, national origin, sex, handicap or ethnicity.
2. The Right to considerate and respectful treatment.
3. The Right to be told in words you can understand what we are doing to help you.
4. The Right to all information necessary to understand your treatment and the right to refuse treatment.
5. The Right to privacy regarding your being here, and the treatment you receive here. You should know, however, that volunteers, students, visitors and professional staff workers from community agencies may visit your unit, but may not enter your bedroom without your consent.
6. The Right to private telephone calls and the receiving and sending of uncensored mail.
7. The Right to expect that your records and what you say here will be kept private. You or your guardian, if you have one, must give your written consent before your records can be shown to other persons authorized by law.
8. The Right to expect that staff will try to do what needs to be done to help you with your problems, including transfer to a more suitable program in another center, if that is appropriate.
9. The Right to know of arrangements made between Chicago-Read Mental Health Center and other health care and educational institutions, if they involve you.
10. The Right to information about the qualifications of the persons who are treating you.
11. The Right to refuse to participate in research projects.
12. The Right to expect that staff will help you find continued care after you leave here.
13. The Right to know the rules and regulations which apply to your conduct while you are here.
